How to Apply for Membership

The Suzuki Music Association of California San Diego Branch welcomes your application for membership. Here’s how to apply:

1. Download printable Applications here in PDF format. You'll need Adobe Reader to view and print the forms (available free from Adobe here).
    
   
2. Complete this application in its entirety. Be sure to sign and date the application.
    
   
3. Please submit correct payment. Please note that your application cannot be processed without payment, and fees are non-refundable.
    
   
4. Annual Membership fees are due each year by August 1st. Return your completed application and payment to the address printed on the form.
